This 10 page questionnaire is intended for teachers of the deaf and/or speech language pathologist in order to gather information from families and caregivers regarding their DHH child. Unlike other questionnaires out there, this one includes questions to gather information about the child's abilities in both ASL and spoken English, as well as information from the family about their own ASL proficiency. An excellent resource for initial and triennial assessments!
